Your appointment time has been reserved for you. Should a need arise to reschedule your appointment, we require 2 business days notice. Please contact us directly during business hours as we cannot accept changes electronically or through our voicemail. We reserve the right to charge a fee for missed appointments or last minute cancellations.

Payment for treatment is due upon completion of the treatment, and is the patient’s responsibility. We are happy to accept assignment from most insurance companies so long as electronic submission is accepted. In situations where electronic submission and coordination of benefits is not possible, the patient is responsible for payment.
